What to Do Prior to Your First Visit

Prior to the Initial Visit

Upon booking an appointment, we will ask you to download and fill out the initial consultation package that will include an in-depth “Health History” form and “Consent” form. This package must be completed prior to your first visit, and brought with you to your first visit. These forms can be downloaded from the links above or picked up at the clinic prior to your visit.

How to Prepare for Your First Visit

Come prepared to share your complete medical history. Your chief complaint is very important and often is connected to various health issues from the past. The comprehensive health history and concerns discussed during the initial visit will help the physicians obtain a holistic picture of your health. If you are currently taking any supplements or medications, please write out all of the pertinent information (brand/generic name, type, potency, dosage, etc.) and bring it with you. If you have any available medical records or lab results from your past, please bring them as well. Come with any questions you may have and, most importantly, be prepared to be an active participant in your own healthcare.

What to Expect from your First and Subsequent Visits

Initial Visits

The initial visit will include an extensive health history with a focus on your chief complaint, any pertinent physical exam, and a determination if further diagnostic tests are necessary. If further laboratory information is necessary, this will be completed between the initial visit and the first return visit.

First Return Visit

At this visit the physician will present his treatment plan with rationale based on all the collected information. At that time, you and Naturopathic physician will continue the journey to optimal health and healing with the commencement of your individualized treatment program.

Subsequent and Additional Return Visits

After your first return visit, the frequency and duration of return visits will dependent on your specific program, but may range from 10 minutes to several hour.
